Community Nursing

Geneva Healthcare has a proud tradition of serving New Zealand communities. Our Community Health nursing team provides a holistic and restorative care service to clients and their whanau – across the lifespan and along the health-illness continuum. 

We are ever flexible, responsive and culturally sensitive in our approach, committed to meeting our clients’ evolving needs, and optimising the level of client functioning and wellbeing to ensure achievable outcomes.

We are guided by fundamental principles of health promotion and maintenance, illness and injury prevention and health restoration, underscored by an emphasis on community participation, and development and empowerment through intersectoral collaboration.

Geneva Healthcare actively embraces the twin challenges of an ageing population and the rising incidence of long-term conditions through:

  • A concentration on a ‘wellness’ model of care delivery, rather than just treating illness/symptoms
  • Aiding clients to receive care in a timely manner and in a client’s choice setting
  • Developing services that are evidence based, safe and embedded in the community and client’s culture
  • Strong nursing leadership and clinical governance, which ensures a systematic approach to maintaining and improving the quality of patient care

  • Supporting people to access and co-ordinate health and social care services when necessary

The Geneva Community Nursing Service assists with:

  • Wound care: acute and chronic management – ACC and Non ACC
    • Vascular assessments including Doppler assessments in the client’s home or a GP practice setting
  • Medication management including IV medications for ACC and Non ACC conditions
  • Continence management including urinary and/or bowel function
  • Assessment of health status including blood pressure and assistance with diabetes management
  • Palliative Care
  • ACC Community Nursing Assessments (CNAs)
  • Referrals for Aged Care Assessment and other health care professionals
  • Education for patients and carers on health conditions including prevention, assessment and management plans 
  • Commitment to nursing educational programme
